Nursing Home Service Line
Community Health physicians and nurse practitioners work in collaboration with nursing home staff. Our physicians and nurse practitioners determine what medical care nursing home patients need. Onsite providers and social workers support and provide daily medical care and oversight to the nursing home patients. They are also responsible for admissions, discharges, diagnosing an illness or condition, assisting with sick care, and providing regulatory visits required by state and federal law.
We are onsite at three Rutland nursing homes
The Nursing Home Service Line is onsite at Mountain View Center, Rutland Healthcare & Rehabilitation Center, and The Pines. Community Health providers see nursing home patients each day and work in conjunction with Community Health nurse practitioners and care coordinators who manage patient ongoing care.
We assist patients with transitions of care
The Nursing Home Service Line assists patients as they are admitted and discharged from a hospital or a nursing home facility. For many patients, a nursing home becomes their home. The Community Health Nursing Home Service Line provides continuity of care as patients have the ability to see a Community Health provider and receive ongoing medical care while in the nursing home.
Nurse practitioners are available 24 hours per day, seven days a week
If a Community Health provider is not present in the nursing home, a designated person is on call 24 hours-a-day, seven days a week.
