All of us at Community Health look forward to taking care of the medical and dental needs of your entire family, something we’ve been doing since 2004. We operate Primary Care Medical Offices in Rutland, Brandon, Castleton, Shoreham, and West Pawlet, Vermont.

Community Health is committed to providing you and your family with accessible, quality, comprehensive, culturally sensitive, evidence-based healthcare.

We participate with most area insurance companies including Medicare and Medicaid and some insurance from neighboring New York State.

We also offer a Sliding Fee Program to our patients who do not have medical insurance. The discount program is based on your income and family size. Please call one of our offices to make an appointment to find out if you qualify. One of our Patient Navigators will be happy to assist you.

Our Primary Care Offices offer a full range of services including:

  • Preventative health care (annual physicals, work physicals, well-child visits)
  • Women’s healthcare
  • Well-child care including immunizations
  • Management of chronic or serious illness
  • Minor office procedures (including laceration repair and removal of skin lesions)
  • Workers comp injuries
  • X-ray and laboratory services are available at some of our locations
  • Express Care

Hospital care for Community Health patients is provided by the Rutland Regional Medical Center’s Hospital Medicine Program. Community Health Pediatrics provides hospital care for pediatric patients.

What is a Federally Qualified Health Center (FQHC)?

Federally Qualified Health Centers (FQHCs) are community-based organizations that provide comprehensive primary care and preventive care, including health, oral, and mental health/substance abuse services to persons of all ages, regardless of their ability to pay or their health insurance status.

A Federally Qualified Health Center (FQHC) is a reimbursement designation from the Bureau of Primary Health Care and the Centers for Medicare and Medicaid Services of the United States Department of Health and Human Services. This designation is significant for several health programs funded under the Health Center Consolidation Act (Section 330 of the Public Health Service Act).

First established in 1965 as part of President Lyndon Johnson’s War on Poverty, FQHCs provide high quality, cost-effective health care regardless of an individual’s ability to pay. The success of the FQHCs was recognized by Congress when creating the Affordable Care Act (ACA) in 2010 and the ACA legislation permanently authorized the program.

Their mission has changed since their founding. Their mission now is to enhance primary care services in under-served urban and rural communities. In particular, they serve under-served, under-insured, and uninsured Americans, including migrant workers and non-U.S. citizens.

Community Health, based in Rutland, is the largest of the 12 FQHCs in the state of Vermont. Nationally, more than 1,200 FQHCs provide primary health care to one out of every 14 people. In Vermont, FQHCs provide care to approximately one in four Vermonters.
