  • In Vermont: 12 FQHCs provide care to approximately 1 in 4 Vermonters

At Community Health:

  • We offer 7 medical facilities and 2 dental practices in Rutland and southern Addison counties.
  • We offer Behavioral Health services at all practices.
  • We offer Express Care for minor illness & injury, 7 days a week, 8am-8pm, at Castleton & Rutland.
  • We provide care to approximately 75% of Rutland County residents.
  • We serve all patients without regard to income or insurance status and provide a sliding fee discount.

In 2019, Community Health provided:

  • 187,848 Total patient visits
  • 41,880 Individual patients
    • 155,026 Medical visits
    • 14,550 Dental visits
    • 15,305 Behavioral Health visits
    • 1,987 Other visits (case management/outreach/diabetes education)
