To our Self Pay Patients: We’re here to help.

Big Saving on Medical Services

  • You will automatically receive a 20% discount because you do not have medical insurance.
  • If you are able to pay your bill at the time of service, you will receive an additional 15% discount.
  • That is a total of a 35% discount from our regular charges if you are able to pay at the time of service.

Big Saving on Dental Services

  • Dental Self Pay is 20% discount if paid on date of service.

Payment Plans Available

In addition, if you set up a payment plan through our patient portal, the monthly amount that you choose to pay on your account can be automatically taken from your credit or debit card. If you need assistance, please speak with Financial Services at (802) 468-2928 or (800) 468-9118.

If you don’t have health insurance

Vermont Health Connect is Vermont’s health insurance exchange that provides individuals, families and small businesses in Vermont with coverage plan options from two medical insurance companies and one dental insurance company.

  • MVP Healthcare
  • Delta Dental

Who can purchase insurance from Vermont Health Connect?

Those eligible to purchase insurance from Vermont Health Connect include Vermonters who:

  • Do not have health insurance
  • Have Medicaid or Dr. Dynasaur
  • Can’t afford the coverage offered by their employers
  • Are small businesses in Vermont that provide coverage for their employees

An Insurance Navigator can explain the different health plans and state financial support you qualify for, including tax credits and other financial assistance, to help pay for healthcare for you and your family (see Insurance Navigators on this page in highlighted box).

Medicaid and Dr. Dynasaur and low cost or free healthcare

Green Mountain Care provides low cost or free medical and dental healthcare coverage for Vermonters through Medicaid and Dr. Dynasaur programs.Eligibility for these programs is based on household income and family size.

Medicaid provides medical, behavioral health, and dental coverage for children, young adults under age 21, parents, pregnant women, caretaker relatives, people who are blind or disabled, and those age 65 and older. Dr. Dynasaur is coverage for children, teens under age 18 and pregnant women.

Frequently Asked Questions

If I am covered by Medicaid, am I also eligible for sliding scale payments?

No. Those covered by Medicaid are not eligible for sliding scale payment plans.

Is there an enrollment period for Medicaid?

No. Enrollment is open year-round for VT Medicaid.

Is there an enrollment period for Vermont Health Connect?

Yes. The annual enrollment period for Vermont Health Connect runs from November 1st through January 15th each year. Check the Vermont Health Connect website VermontHealthConnect.gov for details.

Insurance Navigators

Regardless of your income or insurance status, Community Health serves all patients within our Community Health locations and provides you with assistance and information about medical, behavioral health, dental insurance, and payment options.

Community Health Insurance Navigators assist Community Health patients with understanding the healthcare insurance options available to them. The Navigators are certified by the State of Vermont and are accessible to all Community Health patients at all our locations, either by phone or in person.

Health Insurance and other options

If you don’t have health insurance, an Insurance Navigator can help you with any of the following:

  • Filling out an application for Vermont’s state-sponsored program – Vermont Health Connect
  • Applying for eligibility for Vermont Medicaid
  • Applying for Community Health’s sliding scale payments plans for medical and dental coverage

Community Health patients with questions about Medicare insurance and Medicare supplemental programs can contact the Rutland County Council on Aging. Their Medicare specialists can provide information about how Medicare works and the different coverage available for those 65 and older.

Generally, Medicare is for people 65 or older. However, you may be able to get Medicare earlier if you have a disability, End-Stage Renal Disease (permanent kidney failure requiring dialysis or a transplant), or ALS (also called Lou Gehrig’s disease).

Applying for Medicaid

You will need to meet certain income requirements to be eligible for Medicaid coverage. To complete a Medicaid application, contact our Financial Services Department for assistance. Financial Services will review with you the list of the documents you will need to provide for the application.

If you qualify for Medicaid coverage, the navigator will enroll you in your state plan. If you are not eligible for Medicaid, the navigator will review the coverage options available. They can also refer you to Community Health’s sliding fee scale discount payment plans.

Enrollment in Vermont Health Connect coverage can only be done during the open enrollment period. These are the other qualifying events that would allow you to enroll year-round:

  • Aging out of parental coverage or foster care
  • Birth or adoption
  • Change in income resulting in change in APTC/CSR eligibility (existing enrollees only)
    • APTC: advance premium tax credit
    • CSR: cost-sharing reduction
  • Change in legal status (citizenship, immigration eligibility, lawfully present)
  • Court-ordered coverage
  • Death of a dependent
  • Employer-sponsored insurance (ESI) is deemed unaffordable or inadequate
  • Hardship exemption approved or expires
  • Household member becomes pregnant (new enrollees only)
  • Loss of minimum essential coverage (MEC), employer-sponsored or public plan
  • Marriage/divorce/annulment
  • Moves permanently to the state
  • Native Americans (status allows enrollment anytime)
  • Release from incarceration

Making an appointment with an Insurance Navigator

An Insurance Navigator will assist you with applications for health coverage and determining if you are eligible for one of the financial assistance programs.

During a one-hour appointment, the navigator will review your financial status and medical needs including medications, specialists and chronic conditions. Navigators review the coverage plans available to you but will not make specific recommendations as to which plan you should choose.
