How to prepare for an appointment with your provider

How often have you left an appointment and realized you forgot to ask something? Or worse yet, you left confused about steps to take to improve your condition?

Come to your appointment prepared with this information:

  • Your family history. Whether a parent or sibling had heart disease or cancer and when, is helpful to understanding your risk and can help guide your plan for treatment.
  • A list of medications you take. Including dosage and why you take them.
  • Prioritize and write down your questions and concerns. What are your symptoms? When did they start? What, if anything, relieves the discomfort.
  • Bring a friend or family member. They can remind you what you want to discuss, ask questions, and help take notes.

Write down what your provider says so you can refer to it later. Attach them to the visit summary you receive at checkout.
