Community Health’s Geriatrics program provides specialized, patient-centered care for older adults with complex medical, functional, cognitive, or social needs. Our team works alongside your primary care provider to evaluate concerns, stabilize conditions, and create a clear, personalized care plan so you can maintain your health and quality of life. Once care goals are met, patients transition back to their primary care provider with a coordinated plan for ongoing support.
What to Expect
Your care begins with a comprehensive evaluation of your overall health, including:

- Memory and cognitive health
- Mobility, balance, and fall risk
- Emotional and social well-being
Together, we develop a personalized plan focused on prevention, safety, and managing chronic conditions.

Our interdisciplinary team provides:
- Comprehensive geriatric assessments
- Annual wellness visits and preventive care planning
- Cognitive, mood, and fall risk screenings
- Medication review and support to reduce risks
- Care management and coordination
- Advance care planning and goals-of-care discussions
- Home safety support and post-hospital follow-up
- Referrals to specialty care and community resources
Who May Benefit
This program is designed for Community Health patients ages 60 and older, especially those experiencing:
- Changes in memory or thinking
- Falls or balance concerns
- Difficulty with daily activities
- Multiple chronic conditions
- Complex medication needs
Referrals are made through your primary care provider.
