Brianna L. Hoar, APRN

Brianna Hoar, APRN, FNP-C, has been caring for patients in the Rutland community since 2013. She began her healthcare career as a nursing assistant before earning her Associate Degree in Nursing from Castleton State College in 2015 and later a Master of Science in Nursing from Simmons University in 2021. Throughout her career, Brianna has served in a variety of nursing roles, including leading nursing teams. As a Nurse Practitioner with Community Health, Brianna provides care to residents in the skilled nursing facilities served by our organization. She is passionate about building meaningful relationships with patients and believes that understanding each person’s unique story leads to more holistic, patient-centered care. Brianna is committed to helping older adults maintain their health, comfort, and quality of life through compassionate, individualized care.
