Children with Special Health Needs provides a range of services to Vermont children – birth to age 21 – who have complex health conditions, and to their families. CSHN provides information, referral , medical services, care coordination, and resources to help families support their children’s well being, growth, and development. CSHN services begin with Newborn Screening and continue throughout childhood. CSHN also oversees the Children’s Personal Care and Hi-Tech Nursing Programs and collaborates to oversee the Children’s Palliative Care Program. At the community level, CSHN partners with Children’s Integrated Services Early Intervention (CIS-EI) and the Community Health Teams of the Vermont Blueprint for Health.
