When we are closed, our answering service can be reached by calling the site phone number. Your call will be directed to the appropriate resource for further follow-up. Click here for locations locations.
For any life-threatening conditions, please call 911 and/or go immediately to your nearest Emergency Department.

You can conveniently make appointments with a Community Health provider by calling the Patient Access Center at 802-779-9169.
You also can request an appointment through our secure online Patient Portal.
All Community Health Locations and all Laboratory/Blood Draw Stations are closed to observe the following holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day.
