Folake Azeez, APRN, is a board-certified psychiatric mental health nurse practitioner. She’s a dedicated behavioral health professional with a 22-year career in the field of psychiatry and mental health. Azeez joined Community Health from HCA Houston Healthcare where she worked with patients in the Emergency Department and Inpatient Psychiatric Unit for 11 years.

Philosophy of Care

I practice a “Care Like Family” philosophy in which I strive to treat each patient with respect and dignity and care for them like family.
