Patient Portal
Do you wish you could request prescription refills anytime you want? Would you like to receive your clinical visit summaries electronically and save a tree? Do you want to be able to message your provider in the middle of the night?
Now you can and we’ve made it even easier. We can now email the activation code and link to you, streamlining the process and allowing you to activate your portal immediately.
Through the portal, patients can send messages, access test results, visit summaries, medical history, medications, bloodwork and imaging results and request refills.
The portal can be challenging to navigate, and sometimes Community Health patients have told us that they struggle with it. But we are here to help. We monitor the portal all day long, so if you need a new access code if you are locked out, we respond quickly.
We even provide in-person training. Any time you need training, we’ll spend as much time as is needed so you feel comfortable accessing your medical information.
The portal saves time and gives the patient more control. It replaces the need for patients to come in and sign a release or print a release and sign it so we can provide them with their medical records. If you have a patient portal, it’s just a phone call to us.
