Welcome to Community Health Centers of the Rutland Region. We look forward to taking care of the medical needs for you and your family. We operate Primary Care Medical Offices in Rutland, Brandon Castleton and West Pawlet VT. All of our offices are accepting new patients and we participate with most area insurance companies including Medicare and Medicaid and some insurances from neighboring New York State. We also offer Sliding Fee Scales to our patients who do not have medical insurance. The sliding scales are based on your income and family size. Please call one of our offices to make an appointment to find out if you qualify. One of our Outreach Coordinators will be happy to assist you. Our Primary Care Offices offer a full range of services including preventative health care (annual physicals, work physicals, school, camp and sports physicals), women’s health care, well-child care including immunizations, management of chronic or serious illness, minor office procedures (including laceration repair and removal of skin lesions), workers comp injuries. X-ray and laboratory services are available at some of our locations. Hospital care for CHCRR patients is provided by the RRMC Hospitalists. CHCRR Pediatrics provides hospital care for pediatric patients.

Our new Patient Portal (Follow My Health) is now available.
Please make sure that we have your current email address on file on your CHCRR account so we will be able to send you an invitation to open your Follow My Health (FMH) account. The next time you visit one of our offices, ask one of the receptionists to make sure your email address is correct or you can enter it yourself by using one of the Phreesia tablets while you are in the waiting room.
You will be able to request an appointment or a prescription refill through the portal, as well as pay your bill on line. You can send a general message to your provider and their nurse through the portal. You will also be able to receive your updated health information directly from your CHCRR medical record into your secure Patient Portal account, and you will be able to update your demographic information, such as a phone number or address change or new insurance information from the comfort of your home through your Follow My Health account and it will transfer into your CHCRR account.
As you set up your own Patient Portal account, you will set up and manage your own user name and password information.

Vermont Health Connect - Open Enrollment
Community Health Centers of the Rutland Region is working with Vermont Health Connect as a Navigator Organization to assist community members in Rutland County to navigate the program choices and options and to submit applications.
We have Certified Navigators at each of our health centers.
