Community Health Michelle Wade Named Vermont’s Top Nurse Practitioner

Wade Receives 2018 Nurse Practitioner Award for Excellence, To Be Honored at National AANP Conference

Rutland, VT – Michelle Wade APRN defines excellence. As one of Community Health Centers of the Rutland Region’s (CHCRR) Nurse Practitioners she is known for her thorough understanding of how critically important the work of a nurse practitioner is in caring for the elderly and the most vulnerable patients.  CHCRR recognizes Wade’s unique skills, and now the American Association of Nurse Practitioners (AANP) does too!  AANP named her recipient of the 2018 Award for Excellence in Vermont, naming her the top nurse practitioner in the state. She’ll receive the award this summer in Denver, Co. during the AANP’s National Conference June 26-July 1 when statewide nurse practitioners and advocates will be honored from states around the country.

Wade has put into practice her expertise in caring for the elderly and nursing home patients and helped design a transitional care program for patients during the most difficult times in their care. In nursing homes, she works with sub-acute rehabilitation patients and their families, teaching them how to understand their conditions and how to care for themselves once they are back at home. Wade also works with patients needing long-term, palliative and hospice care.

Wade has presented at national and international conferences and, as a member of the Vermont Nurse Practitioner’s Association, she serves on the policy and conference committees. She also volunteers as a preceptor for nurse practitioner students.

The East Wallingford, Vermont resident earned her nurse practitioner degree from Felician University and bachelor’s and master’s degrees of science in nursing from the University of Phoenix. She is certified by the American Academy Nurse Practitioners Certification Board as an adult-gerontology nurse practitioner.

The State Award for Nurse Practitioner Excellence, founded in 1991, recognizes a nurse practitioner (NP) who demonstrates excellence in practice.  CHCRR proudly announces that the 2018 Award for Excellence belongs to CHCRR Lead Nurse Practitioner Michelle Wade!

CHCRR is central Vermont’s largest network of primary care, pediatric and dental services with offices in Rutland, Brandon, Castleton, West Pawlet and Shoreham. Community Dental offices are located in Rutland and Shoreham, CHCRR Pediatrics is in Rutland and CHCRR’s Express Care centers, open 7 days a week, are located at the Rutland and Castleton Community Health Centers.
