Advance Directive – A Guide For Health Care Decisions

Rutland Community Collaborative Sponsors Free Webinar

Advance planning can bring peace of mind. Detailing your loved one’s health care desires. Leaning how to create your own advance directive. These are the goals of the Rutland Community Collaborative’s second free webinar, “Advance Care Planning for Peace of Mind” on Thursday, January 13, 2022 from 5:00-6:30 pm.

During the discussion and Q&A, a panel of local experts will walk through the process of creating an advance directive and discuss the importance of this valuable and caring document for yourself, a relative or friend.

The panel includes:

  • Eva Zivitz, MSN, RN, CHPN from the Palliative Care Program at Rutland Regional Medical Center
  • Cynthia Stadler, MSN, APRN, CHPN from BAYADA Home Health Care
  • Jessica Greco, LICSW from VNA & Hospice of the Southwest Region

So, what is an advance directive?

An advance directive documents the name of the person who you would like to make health care decisions for you when or if you are unable to make them yourself. The directive also gives instructions about what type of health care you want or do not want.
