Sliding Fee Program

The CHCRR Community Dental Office is pleased to offer you and your family the opportunity to utilize our federally funded Sliding Fee Program.  Community Dental has two requirements prior to a patient applying for the Sliding Fee Program: first, the patient must reside in Rutland County and second, they must apply for Medicaid prior to the application. The Sliding Fee Program is based upon yearly household income; it offers a 50% level and a 100% level (description below). To maintain eligibility, a sliding fee application must be updated every year. 

100% Sliding Fee Program:
The 100% Sliding Fee Program requires the patient to pay a $20.00 co-pay at the time service is rendered. The $20 co-pay covers two (2) procedures; if more procedures are needed an additional $20 co-pay will be collected. The co-pay includes, but is not limited to:

Preventative Services: Exams, radiographs, and dental cleanings
Basic Restorative services: Amalgam and resin bonded fillings
Oral Surgery: Extractions

Both Programs:
Both the 100% and 50% Sliding Fee Programs offer a 50% courtesy discount on major dental procedures and procedures that require lab fees.
These procedures include but are not limited to:
Complete Denture, Metal Framework/Flexible Partial Denture, Interim Partial Denture (1-3 teeth)
Root Canal Therapy
Crowns and Bridges

50% Sliding Fee Program
The 50% Sliding Fee Program requires the patient to pay 50% of the rendered services fees. All services are available for those who qualify.
