Community Health operates 6 medical facilities in west central Vermont in the towns of Castleton, West Pawlet, Brandon, Shoreham, and Rutland, as well as a full-service pharmacy. We provide approximately 172,000 patient visits annually through 50 providers on staff.

Community Health is the predominant primary care provider in the service area, providing approximately 80% of the total primary care. We use a fully integrated practice management system and electronic medical record system and we have a high degree of electronic integration with area hospitals and other providers.

We offer extensive services spanning primary care, behavioral health, dental care, lab services, Express Care, and a full-service pharmacy.
